Home Care Client Care Coordinator

The Client Care Coordinator helps ensure continuity of care for clients after referral and facilitates the provision of in-home care and support services for clients. He or she arranges for the uninterrupted continuing care for clients and helps organize family resources for ensure effective care of the client. Salaried position to include care coordination, in-home assessments and plan of care development and implementation, problem solving and in-home visits. The Client Care Coordinator routinely interacts with other personnel as well as with clients, family members, community providers/referral sources, insurance companies, care managers, and other third party payors.

The ideal candidate for this position has a high school diploma or GED, and at least two years of experience in a health care setting is preferred. Bachelor’s Degree recommended but not required. Experience coordinating health care highly preferred. He or she must have strong organizational and multi-tasking skills, proficiency with computers and business software, and excellent interpersonal and communication skills are needed.  Valid driver’s license as well as properly registered and insured vehicle required.  Fluent English speaking and writing skills, empathy and the ability to listen, good time management skills, and professional dress, speech, and behavior are all required for this position.


  • Visits prospective clients/clients after referrals are made to introduce our company and services
  • Handles prospective client inquires/in-take calls, establishes initial client records, and determines type and duration of services necessary to meet client needs
  • Visits clients to keep abreast of client’s condition and environment at least monthly
  • Helps match caregiver qualifications and availability to clients needs
  • Contacts clients and caregivers for follow-up on service delivery and determines satisfaction of services
  • Serves as a liaison between the client, caregiver, and scheduling/human resources staff
  • May serve on committees and participate in the development of systems to improve care coordination
  • Participates in on-call rotation as assigned
  • Participates in client conferences as requested by supervisor
  • May work with other community resources to ensure that client needs are adequately addressed
  • Performs other duties as assigned

  • High school diploma or GED required; some post high school education or training preferred
  • Excellent computer and Microsoft Office skills
  • Excellent communication skills, verbal and written
  • Unrestricted driver's license and clean driving record
  • Reliable transportation and valid automobile insurance
  • Willing to travel in personal vehicle to visit clients and caregivers
  • Able to maintain satisfactory relationships with clients, family, and team members
  • Able to maintain a high level of confidentiality
  • Neat, clean, and professional appearance
  • Able to stand, sit, walk, reach, kneel, crouch, and lift up to 25 pounds
  • Two years of experience in a health care setting and/or experience coordinating health care preferred
